New Forms Required for Verification of Immigration Documents with the Department of Homeland Security (DHS), 2015

The Department of Homeland Security has announced that, effective October 1, 2015, it will be accepting only the new G-845 and G-845 Supplement Forms, dated “5/20/2015,” with the exception of forms annotated “Third Step Verification Request” or “Do Not Reject-Enumeration” at the top of the form.

The G-845 and G-845 Supplement Forms are used to confirm the immigration status of an individual applying for federal, state or local public benefits, including eligibility for benefits under Titles II, XVI, and XVIII. Typically, the G-845 and G-845 Supplement Forms are submitted together to prove an individual’s legal immigration status in the United States.
